How To Register For NIS
|

You can register in person or by mail. You can download the registration forms below. You
can download the Adobe PDF Form-fill forms and complete them on your computer or you can download the Adobe PDF version for
handwriting the registration information. Registration
forms can be turned in the following three ways: - At a
scheduled regsitration,
- By mail (the address is at the bottom
of each registration form), or
- At my law office between
8:30am and 5:00pm on weekdays (2175 Northdale Blvd. NW, Coon Rapids, MN 55433)
Each registration must be fully completed and signed with the full payment before it will be considered
recieved. If you are registering a team for the U16
& U19 League, the Open Adult League or the Old Fart Adult League, the team must submit a team form along with each players
individual registration form. It is highly recommended that each team collects their players individual forms to reduce
the chance that a player you are counting on has not registered with another team or registers late. Every year we have
multiple teams listing players that are also listed on another team's roster. This creates confusion for everyone involved. IMPORTANT NOTICE: This years registration deadline
has been moved up to February 8, 2012. All forms recieved after this date will be considered late. Late registrations
trigger a late fee and we are no longer able to meet player requests at the U10, U12 and U14 levels. Last year over
1/2 of our registrations were received in the last two days of the registration period or were late. As a result, it
was extremely difficult to get the teams rosters together, the schedules created, the shirts ordered in time for the coaches
meeting. We understand that many people want to wait as long as possible before turning in thier registration forms
but we simply don't have enough people to deal with 750 registration forms, 8 leagues and over 50 teams in such a short period
of time so we have decided to move the registration deadline up. WE DON'T WANT TO COLLECT ANY LATE REGISTRATION
FEES, WE JUST WANT THE REGISTRATIONS TO BE RECIEVED IN TIME. |
Registrations will be held at the Nash Law
Firm offices located at 2175 Northdale Blvd NW, Coon Rapids, MN 55433 at the following times and dates: - Saturday, January 28 11am to 2pm
- Saturday, February
4 11am to 2pm
- Wednesday, February 8
6pm to 8pm
